Our client who is an expert in their field are looking for an experienced Rehabilitation Case Manager to join their team on a full time basis.

This is a hybrid role where you will be expected to travel when visiting employees and clients.

Main duties and responsibilities:

  • Managing a case load of clients through their rehabilitation.
  • Managing all administration in relation to cases.
  • Assessing what needs to be in place for the people you support to live a fulfilled life.
  • Writing and reviewing reports.

Qualifications and skills:

  • ​Clinically qualified.
  • Minimum of 2 years experience in an operational management capacity in a health and social setting.
  • Experience of managing a complex care service.
  • Knowledge and understanding of current legal responsibilities and standards of the service. 
  • Full, clean driving license.
